Neurologic Rehabilitation

Mobile care delivered where it matters most, in your home and daily environment.

  • A comprehensive assessment will be done with a Doctor of Physical Therapy to identify your current challenges and functional level, establish meaningful and personal goals, and begin building a plan that supports your next phase of progress. The PT will come to you and provide this assessment in your home environment and leave with an evidence-based plan for your first steps forward.

  • Ongoing sessions are tailored to you, your goals, your environment, and utilize your preferences along with expertise and evidence-based practice. Care is delivered one-on-one, without distractions, so you feel supported.

  • A brief discovery phone call to discuss your concerns, determine if this is a good fit, and answer any questions about care.
